FMLA Assessment

An FMLA (Family and Medical Leave Act) Assessment is a comprehensive evaluation to determine whether a medical or mental health condition is significantly impacting your ability to perform essential job functions and may qualify for protected leave under the Family and Medical Leave Act. During this assessment, we will review your symptoms, functional limitations, treatment history, and the ways your condition affects your daily life and work performance.

If clinically appropriate, recommendations regarding leave, workplace accommodations, or intermittent absences will be discussed. Documentation can be completed based on the findings of the assessment and in accordance with professional and legal standards. The goal of this evaluation is to ensure that your needs are accurately assessed and that appropriate support is provided to promote your health, recovery, and overall well-being.
